PENDEKATAN MULTIMODAL FISIOTERAPI UNTUK LOW BACK PAIN: STUDI KASUS KOMBINASI SHORT-WAVE DIATHERMY, TENS, DAN LATIHAN

Abstract

Low back pain (LBP) merupakan salah satu gangguan muskuloskeletal yang paling umum dengan prevalensi seumur hidup mencapai 60–70%, menyebabkan beban sosial dan ekonomi signifikan. Modalitas fisioterapi seperti Short Wave Diathermy (SWD), Transcutaneous Electrical Nerve Stimulation (TENS), dan McKenzie exercise sering digunakan, namun kombinasi protokol optimal pada praktik klinis memerlukan eksplorasi lebih lanjut. Laporan kasus deskriptif ini melibatkan seorang pria 35 tahun dengan LBP kronis (>2 bulan) pada level L4–L5, VAS ≥5, ODI ≥30%, dan MMT ≤3 tanpa kontraindikasi SWD/TENS. Pasien menerima 3 sesi kombinasi SWD (15 menit, intensitas 70% toleransi), TENS (100 Hz, 20 menit), dan McKenzie exercise (3 set × 10 repetisi). Outcome diukur menggunakan VAS, MMT, ROM dengan goniometer, ODI, dan SF-36 pada pre-intervensi, mid-intervensi (sesi ke-2), dan post-intervensi (sesi ke-3). VAS istirahat turun dari 6 → 4 → 3; VAS gerak dari 8 → 5 → 4; MMT dari 3 → 3+ → 4; ROM fleksion dari 50° → 59° → 65°; ODI dari 48% → 36% → 28%; SF-36 domain fungsi fisik meningkat +15 poin dan domain nyeri +20 poin. Efek samping ringan tanpa komplikasi serius. Kombinasi SWD, TENS, dan McKenzie exercise secara signifikan mengurangi nyeri, meningkatkan kekuatan otot, jangkauan gerak, dan kualitas hidup pasien LBP. Disarankan penelitian eksperimental dengan sampel lebih besar dan desain RCT.
