Teknologi digital ortodonti lebih sering dinilai dari akurasi pengukuran atau performa algoritme daripada hasil pasien. Tinjauan ini menilai jarak antara validasi teknis dan bukti hasil pasien pada diagnostik tiga dimensi, Dental Monitoring (DM), dan artificial intelligence (AI). MEDLINE/PubMed ditelusuri untuk publikasi 1 Januari 2000 sampai 18 Juni 2026, dengan pencarian dijalankan pada 20 Juli 2026. Satu penelaah melakukan screening dan penilaian kelayakan dengan log keputusan terstruktur. Risiko bias dinilai memakai RoB 2, ROBINS-I, AMSTAR 2, dan ROBIS, serta temuan disintesis secara naratif. Pencarian data menghasilkan 2.498 temuan unik. Screening judul dan abstrak mengeksklusi 2.383 temuan. Sebanyak 115 laporan dinilai pada tahap kelayakan; 72 dikeluarkan dengan alasan tercatat. Sintesis mencakup 43 laporan, terdiri atas 25 studi primer dan 18 systematic review atau meta-analysis. Pemindaian intraoral umumnya lebih nyaman daripada impresi konvensional. DM mengurangi kunjungan, tetapi hasil durasi dan oklusal tidak konsisten. CBCT mengubah diagnosis atau rencana pada kasus terpilih tanpa bukti jelas tentang hasil akhir yang lebih baik. Bukti AI masih berpusat pada akurasi tugas, waktu analisis, dan kesesuaian dengan klinisi. Manfaat bagi pasien paling jelas pada kenyamanan dan pengurangan kunjungan. Bukti superioritas klinis masih terbatas, terutama untuk AI. Pencarian satu basis data dan seleksi oleh satu penelaah menurunkan kepastian.
