Abstract

Diabetes melitus tipe 2 (DMT2) merupakan gangguan metabolik kronis dengan prevalensi global yang meningkat pesat, yang ditandai oleh hiperglikemia persisten akibat resistensi insulin dan disfungsi sel beta pankreas. Pada tingkat molekuler, patogenesisnya berkaitan erat dengan stres oksidatif dan inflamasi kronis yang dimediasi oleh interaksi antara jalur Nuclear factor erythroid 2-related factor 2 (Nrf2) dan Nuclear Factor Kappa B (NF-κB). Penelitian ini bertujuan untuk menganalisis secara komprehensif hubungan silang (crosstalk) antara kedua jalur tersebut melalui tinjauan naratif dengan pencarian literatur terstruktur dari PubMed, ScienceDirect, dan Google Scholar. Hasil kajian menunjukkan bahwa hiperglikemia kronis meningkatkan produksi Reactive Oxygen Species (ROS), yang memicu aktivasi NF-κB serta meningkatkan ekspresi sitokin proinflamasi seperti TNF-α dan IL-6, yang berkontribusi terhadap resistensi insulin dan kerusakan pankreas. Sebaliknya, Nrf2 berperan sebagai regulator antioksidan utama dengan menginduksi enzim seperti glutathione peroxidase; namun, aktivitasnya menurun pada kondisi stres oksidatif yang berkepanjangan. Ketidakseimbangan antara hiperaktivasi NF-κB dan disfungsi Nrf2 ini mempercepat progresivitas penyakit. Sebagai kesimpulan, penargetan keseimbangan antara kedua jalur tersebut merupakan strategi terapeutik yang menjanjikan untuk mencegah komplikasi dan meningkatkan luaran metabolik pada DMT2.
